REMOTE POSITION - MUST RESIDE NEAR ORLANDO, FL AREA

Job Responsibilities:  

The Health Coach will function as a critical member of the Heart Failure Program and will work closely with the heart failure clinical team to provide patient education, guidance, and satisfaction, and ensure that our patients come first.

The Health Coach will be responsible for guiding, educating, and bringing solutions to patients in need. If you are committed to seeing patients improve their health, then this will be a very rewarding experience.  Job duties include, but may not be limited to: 
 
  • Provide patient education on chronic disease management, particularly educating them on their heart failure diagnosis and management.
  • Provide motivational and clearly communicated educational components including heart health, diet, exercise, medication adherence.
  • Establish short-term and long-term goals as part of shared decision-making with the patient and monitor patient progress throughout the program.
  • Oversee educational components of the program and ensure accurate documentation, patient progress and completion
  • Encourage and motivate patient engagement, advocacy and self-development.
  • Follow up on patient communications and return patients’ messages in a timely manner.
  • Aim to deliver an excellent patient experience.  
  • Build a good rapport and maintain long-lasting relationships with patients and caregivers.  
  • Collaborate and work regularly with the clinical team to ensure optimal patient outcomes, including escalating patient concerns in a timely manner, providing patient status updates, and sharing care plan documentation.
  • Participate in interdisciplinary team meetings.
  • Perform other duties and assist team in patient-centered tasks as assigned.

Qualifications and Background:  
  • Health coach certification from a program accredited by the Commission on Accreditation of Allied Health Education Programs (CAAHEP) or the Accrediting Bureau of Health Education Schools (ABHES). A bachelor’s degree in health sciences preferred.
  • Experience in a medical office setting or healthcare environment. Experience in cardiology-related coaching a plus. 
  • Experience with telehealth a plus. 
  • Strong interpersonal skills to communicate effectively and pleasantly with patients and other care team providers.  
  • Excellent customer service skills with the goal of achieving high quality customer satisfaction. 
  • Proficiency in working with electronic health records, such as Athena, as well as tools including Microsoft Word, Excel and other computer applications is strongly preferred. 
  • Bi-lingual fluency (English and Spanish speaking) highly preferred. 
  • This position is remote. A dedicated space to ensure patient privacy with reliable broadband is required.
